Pre-Marital Spiritual Counseling Questionnaire

Marriage is a sacred covenant, designed by God to reflect His love and unity (Ephesians 5:31-32). As you prepare for this lifelong commitment, it is essential to lay a strong spiritual foundation that will sustain your union through every season. This form serves as a prayerful reflection of your relationship, helping me understand your strengths, challenges, and areas where biblical guidance is needed before you say “I do.”

Honest and complete responses are vital, as they allow for Spirit-led discernment, accountability, and preparation. As Psalm 127:1 reminds us, "Except the Lord build the house, they labour in vain that build it." This form is a step toward ensuring that your marriage is built on Christ, wisdom, and mutual understanding.

To ensure prayerful preparation and meaningful counseling, this form must be completed at least 48 hours before your first scheduled session. Your commitment to this process is a reflection of your faith, readiness, and willingness to seek God’s will for your marriage.
